No form, and nothing on this site records anything about the people reading it. A production enquiry arrives with a rider, a stage plan and a venue spec attached (none of which fits in a form field).

What a hire enquiry should include

The information that lets an enquiry be answered rather than guessed at.
ItemWhy it's needed
VenueRoom dimensions, audience layout, flying height and rigging points determine the whole specification.
DatesGet-in, show days and get-out (the hire period, not just the performance date).
AudienceNumber and how they are distributed. A standing floor and a seated rake are different problems.
ProductionWhat is happening on stage, and any technical rider that comes with it.
PowerWhat supply exists, of what rating and type, and where it terminates.
Noise limitsAny limit at a boundary or imposed by a licence (it can change the system design entirely).
CrewWhether labour is required, or whether the hirer is providing it.

Outdoor events

An outdoor event changes almost everything: weather protection for equipment, wind loading on anything flown or on a structure, ground conditions for vehicles and towers, power generation rather than a mains supply, and noise propagation that's far harder to control without walls. Say at the outset if an event is outdoors.

What comes back

Everything gets a reply. If the job is one we can do, a specification and a price follow. If the dates clash, or the scale is wrong, it says so straight away rather than leaving a production waiting on an answer.
